How is Gynecomastia Graded?

Gynecomastia is classified into four grades based on the extent of breast tissue enlargement, skin excess, and chest contour changes. This classification system helps Dr. Deepu determine the most effective surgical approach for each patient. During your consultation, Dr. Deepu will evaluate your grade through physical examination and determine the optimal treatment plan.

Grade 1 Gynecomastia Cross Section

Grade 1 - Mild

Slightly enlarged or puffy nipples with minimal excess fat or glandular tissue concentrated beneath the areola. The chest contour remains largely masculine. This is the most common grade and typically responds well to liposuction alone, sometimes combined with minimal gland excision through the areolar margin.

Grade 2 Gynecomastia Cross Section

Grade 2 - Moderate

Moderately enlarged breast tissue extending beyond the areola boundary with visible fullness and puffy nipples. The chest begins to show a noticeable feminine contour, particularly when wearing fitted clothing. Treatment requires a combination of liposuction and gland excision for optimal results.

Grade 3 Gynecomastia Cross Section

Grade 3 - Significant

Significantly enlarged breast tissue with excess fat and glandular tissue creating a noticeable chest contour change. Mild skin excess may be present. Treatment involves comprehensive liposuction, gland excision, and possible skin tightening techniques to achieve a flat, masculine chest profile.

Grade 4 Gynecomastia Cross Section

Grade 4 - Severe

Prominent, rounded chest with significant excess tissue and skin creating a distinctly feminine appearance. This grade requires the most comprehensive surgical correction including liposuction, complete gland excision, and possible skin removal to eliminate excess skin and achieve a flat, contoured masculine chest.
